Admissions


     The Center will accept children between the ages of six weeks and seven years (first grade). Families with financial hardships cannot be provided with scholarship help by the Center at this time. It is unlawful for the Center to discriminate in the enrollment of children and families upon the basis of race, color, religion, sex, national origin, or disability. Additionally, the Center does not discriminate on the basis of sexual orientation.

     Enrollment will be open first to children requiring full five-day-per-week care. Children receiving part-time care are accepted if space is available, with preference being given to children requiring full-time care. In some instances families of part-time children may be asked to rearrange their schedule, if possible.

     A child will be considered enrolled after the administrators / coordinators have confirmed the availability of a space, the registration fee and security deposit have been received, and the day of orientation has been completed. The following forms must be completed as a part of the enrollment process as well:

  1. Child Enrollment and Health Information Form - contains the names of two persons who are authorized to be called in case of an emergency, as well as permission to transport your child in case of an emergency. Contains complete information for reaching the Parents/Guardians, including work, cell phone and pager numbers. Also lists the information for the family doctor. This form is to be reviewed annually and updated as needed.
  2. Child’s Medical Statement - must be completed, signed and dated by your family doctor. This form is due on the first day of enrollment, and certifies that your child has all of his or her required immunizations. This form needs to be up-dated annually, until your child is enrolled in Kindergarten.
  3. Notes About Your Child - includes information about your child’s allergies, sleeping and eating habits, discipline used at home, fears and interaction with other children.
  4. Family Background Questionnaire - includes information about the child’s family and significant people in his or her life.
  5. Parent/Guardian Receipt of Handbook - certifies that you have read and understand the Parent Handbook, which contains the rules and regulations of the Center.
  6. Tuition Payment Agreement - outlines the weekly tuition amount, the registration fee and the Security Deposit. Indicates orientation date and first day of enrollment, and requires parent/guardian(s) signature and date.
  7. Registration fee: One fee per family ($65.00). A child is not considered enrolled until the registration fee and tuition fee is received.
  8. Security System Memo of Understanding and Guidelines - includes information regarding the security system and receipt of security card acknowledgment.
  9. Medical Care Plan Form (if applicable) - outlines special medical needs and instruction for administering medications. This form needs to be up-dated annually.
  10. Special Diet Form (if applicable) - outlines special diet needs and reasons for restrictions/modifications. This form needs to be up-dated annually.
  11. CACFP Enrollment Form – requests schedule of child & meals/snack child will partake in. This form needs to be up-dated annually.

[PROSPECTIVE PARENTS: Please note: filling out any paper work (the wait list sheet or any downloaded enrollment forms) does not imply that your child(ren) have been accepted for admission. You must first come in for a tour. You will receive a phone call from either the Director, Assistant Director, or Coordinator to set up an Orientation Day. It is at that time you are accepted and fill out the paper work.]
