Admissions
The Center will accept children between the ages of six weeks and seven years (first grade). Families with financial hardships cannot be provided with scholarship help by the Center at this time. It is unlawful for the Center to discriminate in the enrollment of children and families upon the basis of race, color, religion, sex, national origin, or disability. Additionally, the Center does not discriminate on the basis of sexual orientation.
Enrollment will be open first to children requiring full five-day-per-week care. Children receiving part-time care are accepted if space is available, with preference being given to children requiring full-time care. In some instances families of part-time children may be asked to rearrange their schedule, if possible.
A child will be considered enrolled after the administrators / coordinators have confirmed the availability of a space, the registration fee and security deposit have been received, and the day of orientation has been completed.
ORIENTATION AT ROCKPORT EARLY CHILDHOOD CENTER
When your child becomes enrolled at Rockport Early Childhood Center, you will need to spend seven or eight hours in one day or two half days in an orientation period before you leave your child at the Center for the first time. This gives you and your child a chance to meet the teachers and other children and become familiar with the classroom. The teacher can get to know you and your child before leaving him/her for the first time.
During your child’s orientation, we ask that you stay with your child and provide any care needed. You will start paying tuition when you leave your child on the first day of enrollment. You may leave the classroom for a short period, but we ask that you stay in the building. If your child takes a nap, you may leave the classroom at that time, as the teacher prepares the children and room for napping.
Naptime is a good time to go over Enrollment papers in the Office. Payment of the Security Deposit, Registration Fee and First Tuition payment are due on the day of Orientation. You must return all Enrollment papers Office on the first day your child officially attends the Center.

The following forms and items must be completed as a part of the enrollment process as well:
- Child Enrollment and Health Information Form - contains the names of two persons who are authorized to be called in case of an emergency, as well as permission to transport your child in case of an emergency. Contains complete information for reaching the Parents/Guardians, including work, cell phone and pager numbers. Also lists the information for the family doctor. This form is to be reviewed annually and updated as needed.
- Medical Care Plan Form (if applicable) - (01582) outlines special medical needs and instruction for administering medications. This form needs to be up-dated annually.
- Special Diet Form (if applicable) - outlines special diet needs and reasons for restrictions/modifications. This form needs to be up-dated annually.
- Request for Administration of Medication Form (if applicable) - outlines medications your child need to take while at the Center. Please view these documents (01580, 01581) regarding how to properly fill out the forms and to bring in the medications. This form needs to be up-dated annually.
- Child’s Medical Statement - must be completed by child’s physician, signs and dated. You must return the Child Medical Statement to the Office within 30 days after enrollment . You may have the physician’s office fax the form to us at 440-331-3094. This form needs to be up-dated annually, until your child is enrolled in Kindergarten.
- Ages and Stages Questionnaire® Third Edition (ASQ-3™) Permission Slip (if applicable) - must be turned in the first day of enrollment with all other enrollment forms whether or not you agree to have your child screened. (Children in Kindergarten or First Grade are not eligible.)
- File Information Form - a form stating everything you need to fully comply with all Rockport Early Childhood Center and ODJFS Licensing Rules.
- Family Background Questionnaire - includes information about the child’s family and significant people in his or her life.
- Notes About Your Child - includes information about your child’s allergies, sleeping and eating habits, discipline used at home, fears and interaction with other children.
- Safety Policy - includes information about the Center's Safety Policy.
- Security System Memo of Understanding and Guidelines - includes information regarding the security system and receipt of security card acknowledgment.
- Parent/Guardian Receipt of Handbook - certifies that you have read and understand the Parent Handbook, which contains the rules and regulations of the Center.
- Tuition Payment Agreement - outlines the weekly tuition amount, the registration fee and the Security Deposit. Indicates orientation date and first day of enrollment, and requires parent/guardian(s) signature and date.
- Registration fee: One fee per family ($65.00). A child is not considered enrolled until the registration fee, security deposit, and tuition fee is received.
- Security Deposit: equivalent to one week of tuition, minus the $2.00 per week supply fee. A child is not considered enrolled until the registration fee, security deposit, and tuition fee is received.
- First Tuition payment: dependant on the amount of days in that 2-week billing period. A child is not considered enrolled until the registration fee, security deposit, and tuition fee is received.
- CACFP Enrollment Form – requests schedule of child & meals/snack child will partake in. This form needs to be up-dated annually.
- Infant Meals – Parent Preference Form – for infants in Room 22 only.
- Electronic Format SignOff Form – we have converted to the use of e-mail for Newsletters, Invoices, etc.
[PROSPECTIVE PARENTS: Please note: filling out any paper work (the wait list sheet or any downloaded enrollment forms) does not imply that your child(ren) have been accepted for admission. You must first come in for a tour. You will receive a phone call from either the Director or Coordinator to set up an Orientation Day. It is at that time you are accepted and fill out the paper work.]
